This publication is the second report of a project carried out in 2001-02 for the Danish Ministry for the Interior and Health about requirements to a reporting system of adverse events in hospitals. The aim of the project was to work out/draw up a set of recommendations for the development of a system of reporting- and learning from adverse events in hospitals.

The recommendations were based on a review of international experience with reporting systems, interviews with and an extensive questionnaire survey amongst nurses and doctors. This report contains a descriptive overview of the results from a national questionnaire survey of doctors and nurses attitudes toward a registration and reporting system for adverse events in hospitals; staffs and departments handling of adverse events and errors and respondents perspective on potential barriers against reporting.

The survey contains answers from 1305 nurses and 703 doctors from hospitals in Frederiksborg, Copenhagen, Ringkøbing and Roskilde counties. The report contains: • A short description of the background for the questionnaire survey, the development of the questionnaire, collection and sample criteria. • A systematic overview and summary of the questionnaire results that contain doctors and nurses attitudes toward. • Four cases: the degree of openness towards patients in relation to the degree of harm made. • Models of reporting, confidential or anonymous, who should receive and give feedback, and whether reporting should be free or mandatory. • Reasons for not reporting, expectations to current leaders reaction to error, and perspective on the reasons for adverse events. • Patients’ wishes after adverse events. • Reactions towards staff who have been involved in adverse events, and departments handling of error and adverse events • Patient safety and handling of error and adverse events, attitudes towards error, own and colleagues’ performance and competencies. • The thought of making error and questions about supervision.

The report contains: Appendix 1: Tables of the results in percentages for each questions for both doctors and nurses; Appendix 2: Questionnaire (doctors’ version); Appendix 3: Diagrams ranked by nurses’ response data.
